Be identified as having practice issues due to a physical, mental, or chemical dependency (prescribed or illegal drugs) impairment or
Be an individual whose employer has directed him/her to participate in TnPAP as a condition of continuation of employment or
Be an individual who recognizes that (s)he is experiencing difficulty with a physical, mental health or chemical impairment before being confronted by an employer, the police or a Tennessee Department of Health investigator or
Be an individual whose employer or whose licensure board has directed him/her to participate in TnPAP as part of disciplinary process.
Be an individual who is willing to inform any university/college that (s)he is in the process of TnPAP enrollment.
Timely completion of authorization for exchange of information (aka Release of Information) forms as instructed by TnPAP, including but may not be limited to:
After submitting Enrollment Request, call TnPAP and speak to an intake representative who will provide you with information on scheduling an evaluation. You must use an evaluator that is approved by TnPAP.
Schedule an evaluation.
Immediately notify TnPAP of the evaluation appointment date and time. Failure to do so may result in the file being closed.
Complete and submit the following release of information form.
Go to the left menu or home page. Select Forms from the menu list on the upper left of the page. From the list titled "Communication Authorization" (aka Release of Information) select Evaluator or Health Care Provider. Complete and submit.
Once the evaluation recommendation report is received in TnPAP, it is reviewed by the Executive Director and a letter will be prepared and mailed to you (certified) outlining the recommendations, if any.
Follow all evaluations recommended (neuropsychological testing, medical evaluations, pain management assessment, etc.) if any.
Follow all treatment recommendations made, if any.
Be responsible for all costs of evaluation, treatment and toxicology screening.
