Steps in the Enrollment Process
A healthcare provider wishing to utilize TnPAP services must agree to comply with all requests and
recommendations made during the enrollment process. Steps include, but may not be limited to:

Voluntarily cease practice until a return to practice authorization is obtained from TnPAP. |
| Step II |
Complete and submit Enrollment Request within three (3) business days. |
| Step III |
Complete and submit the following release of information forms.

| Step IV |
After submitting Enrollment Request, call TnPAP and speak to an intake representative who will provide you with information on scheduling an evaluation. You must use an evaluator that is approved by TnPAP. |
| Step V |
Schedule an evaluation. |
| Step VI |
Immediately notify TnPAP of the evaluation appointment date and time. Failure to do so may result in the file being closed. |
| Step VII |
Complete and submit the following release of information form.

Once the evaluation recommendation report is received in TnPAP, it is reviewed by the Executive Director and a letter will be prepared and mailed to you (certified) outlining the recommendations, if any. |
| Step IX |
Follow all evaluations recommended (neuropsychological testing, medical evaluations, pain management assessment, etc.) if any. |
| Step X |
Follow all treatment recommendations made, if any. |
| Step XI |
Be responsible for all costs of evaluation, treatment and toxicology screening. |
| Step XII |
Maintain weekly communication with TnPAP. |
