To schedule an orientation date please contact your TnPAP case manager. Also, be sure to request your pin number, which is required to activate your AOS (Affinity Online Solutions, Intl, SRL) account.

 

It will be helpful if you review a monitoring agreement overview and jot down any questions that you want to be sure and ask your case manager. This overview is of a standardized monitoring agreement and may have stipulations that may not be applicable to your specific monitoring agreement. Taking time to review the enclosed information will expedite the orientation process for you.

 

It is required that you register for the TnPAP random toxicology screening program prior to orientation. Once registered, you do not begin daily "check in" until after you have actually signed the monitoring agreement.

 

It is very important that you bring the following items with you:

 

  • Printed documentation of registration in the TnPAP approved random drug screen program. To register with AOS (Affinity Online Solutions, Intl, SRL) go to www.aosintl.com/cms , enter your pin number and proceed with instructions to activate your account.
  • To establish and maintain an account a credit card or gift card should be utilized to make payment. Gift cards can be purchased at banks, most pharmacies and groceries. For more locations go to www.visa.com. The amount will be dependent on the service that you have selected to use:
    • Online service $15 administration fee + $100 UDS prepayment = $115 (no alternative to online check-in available)
    • Full service $50 administration fee + $100 UDS prepayment = $150 (may check-in either online or by phone)
    • If you do use a credit/debit card it is expected that the card will be valid and the amount of the screen and/or laboratory fee will be accepted immediately.

    • The results of a recent (negative) UDS screen (within the past 10 days).
    • Bring the bottles of any medication (prescription and over-the-counter) that you are taking.

     

    • A money order or cashier's check made payable to TNPAP in the amount of $40 for your Recovery Maintenance Workbook (credit cards or cash are not accepted).

    • Personal letter outlining your disease progression. The letter must include a list of your personal relapse warning signs, symptoms and triggers.

     

     

    Individuals attending orientation should report to 545 Mainstream Drive, Suite 414, Nashville, TN on the date scheduled. Give yourself enough time to ensure that you do not arrive late. If you must reschedule, please contact TnPAP so that another date may be scheduled. You will not be allowed to reschedule more than one time.
