Evaluators, Treatment Centers and Support Group Facilitators

Support Group Facilitators and Treatment Providers

Support Group Facilitators are located throughout Tennessee, for use by program participants.  CLICK HERE for list of these facilitators.  Please contact the Intake Case Manager at 615.726.4001 to obtain additional information.

Treatment Facility

Treatment Facility Information may be obtained by calling 615.250.6104 or TnPAP case manager.

Any treatment facility that would like to be added to the TnPAP referral list should have a representative contact the State Director.

Support Group Facilitator

If you are interested in being a support group facilitator, you must meet the following qualifications:

  • Have one of the following:
     
    • Prior experience in group skills and dynamics.

    • A thorough knowledge and understanding of the disease of addiction, chemical dependency, sobriety, recovery, acceptable community treatment standards, 12-step meetings, relapse, and rehabilitation of social and professional life skills.

  • If recovering, shall have been clean and sober for a minimum of four years.

  • If previously a TnPAP participant, shall have successfully completed the assigned program.

  • Must have completed one TnPAP orientation as conducted for new participants.

To apply:

  • Complete an application

  • Complete and sign Confidentiality Statement

  • Attach a current resume

  • Attach a cover letter expressing your desire to become a support group facilitator and willingness to adhere to TnPAP policies.

  • Mail application, confidentiality statement, resume, and cover letter to:
